Forensic Services:

Referrals for forensic evaluations must come from attorneys or case workers.

Please note: I DO accept indigent defense cases, with billing through PDSC (Public Defense Service Commission), at a reduced rate.

Evaluation (e.g., testing, interviewing, record review, report writing) $250 per hour.
Court time $250 per hour, billable in half-day (4-hour) increments.

Therapy Fees:

- Initial evaluation (one hour): $250.00
- Therapy session (45 min.): $165.00
- I accept payment by cash, check, and credit/debit card.

Insurance:

I am a provider for the following insurance and managed care companies:

Blue Cross/Blue Shield PPO, preferred provider
Managed Health Network (MHN)
Providence Preferred

If I am a provider for your health plan, I will bill your insurance directly. At the time of service, you are responsible for co-payment and any deductible.

Other insurance and managed care companies often cover services for therapists who are out-of-network. In such cases, you will need to pay me in full and I will supply you with a receipt in order to be reimbursed by your insurance company.
